What is a "health village and educational facility" I mean, what exactly does that mean, a health village?

as for educational facilitys well most of us would call those schools.
a village is usually a small community where people live

I am not so certain about a "health" village though.

This is not identical to what existed in the past. It's an attempt to build something new for the future which integrates hospital care, training facilities for medical professionals, and community health facilities.
